The issue is likely your GPU is old and is below suggested specs for GameGuru. It will run slow without a GPU upgrade, the rest of your PC appears to be fine.

Yeah, the PC is about 5 years old, but the 560 is above the min (400 series), but below the recommended (660TI). So at min specs 5 fps would be normal?

I was looking at new PC's, but wanted to check if this was just a GPU issue. I read somewhere that once compiled the game runs better, is that what you have seen in your experience?

The current listed min specs for GameGuru really are too low for playable speeds, you won't get playable FPS in test games without having at least the recommended. Currently the engine is being updated to DirectX11, which will mean all the specs will be updated as a result. Meaning the current recommended specs will likely become the min specs after the update is released.

Often after you have compiled your game and added light maps it will run better than a test game but this isn't always the case, as with any game engine you as the designer need to do optimization work to make it run nice and smooth.
